An interesting program in NY was announced last week which with foundation funding will bring together community health centers to, “… help twelve New York City community health centers leverage two historic state and federal financial incentives, while simultaneously transforming the delivery of primary care.”
As a former Chief Medical Officer of Baltimore Medical System, Inc. (a community health center); a native New Yorker, and of course an advocate for appropriate use of health IT to support improvements in patients care, it is gratifying to see the merging of the medical home concept and the EHR Incentive Program.
